Only the person(s) named on the certificate, with a valid photo ID, may request it.
A parent or legal guardian may request one on behalf of a minor.
Certificates are issued in hard copy only with the Church seal and must be picked up in person.
None are sent by mail.
Please allow 1-2 weeks for processing.
This request applies only to Sacraments celebrated at St.Joseph Catholic Church.
A processing fee of $20.00 will apply for each certificate requested.
Once submitted, you will be contacted when the certificate is complete and ready for pick-up.
*Please note, submitting the proper name and date will help expedite the process, if an improper name and/or no sacrament date is submitted, it will further delay the processing of your sacramental certificate request.
