Sacrament Certificate Request

Only the person(s) named on the certificate who the sacrament was for, with a current valid photo ID, may submit a request. A parent/legal guardian can request the certificate if it is for a minor.
All certificates are provided in hard copy only with the church seal. No certificates will be sent via email.
Please allow at least 1-2 weeks to research and provide you with a hard copy sacrament certificate.
A processing fee of $10.00 will apply for each certificate requested.
Request for copy of Baptismal, First Communion, Confirmation or Marriage certificates only applies to St. Joseph Catholic Church. St. Joseph Catholic Church is not responsible for requesting certificates from other Churches.
Once submitted, you will be contacted when the certificate is complete and ready for pick-up

*Please note, submitting the proper name and date will help expedite the process, if an improper name and/or no sacrament date is submitted, it will further delay the processing of your sacramental certificate request.
